Who can report to the Yellow Card scheme?

The Yellow Card scheme relies on voluntary reporting of any problems with a healthcare product by the public (including patients, parents, and carers), as well as by healthcare professionals. You can report for yourself or for someone else, with their permission.

We are unable to provide medical advice. Speak to your doctor, pharmacist or call NHS 111 if you are worried about your health.

Other ways to report to the Yellow Card scheme

The Yellow Card scheme has an app which supports the creation and tracking of reports. Find out more from your apple or android app stores.
