If you cannot complete the online report or form, for reporting, you can call us on 0800 731 6789 (9am to 5pm Monday to Friday). Adverse incidents involving medical devices cannot be reported by telephone. Please report online or by email to aic@mhra.gov.uk

The MHRA works with six regional Yellow Card centres to provide awareness and learning to the Yellow Card scheme for healthcare professionals, patients and representative organisations.
